BELOW IS THE MODEL RELEASE FORM.

Besides giving the Photographer permission for a Model to be photographed, there are other very important items in the Form which a Model absolutely needs to know BEFORE a session begins.

Remember: A Model Release Form is Legal and Binding once it is signed. If a Model has any questions or concerns, it is best for them to go over everything BEFORE it is signed and a photoshoot begins.
